Question: We all have those patients who come to the office with a handwritten list of questions. They never take their shoes off, but spend 40, 50, 60 minutes asking question after question.
Although I think it would be very difficult to justify a 4 or 5 level E/M service, would it be appropriate to bill a 4 or 5 level “counseling” E/M, based on time alone, under the circumstances noted above?
